Black Hawk OFS believes that each employee contributes directly to our growth and success, so we pride ourselves in offering the following benefits package to all full time employees.

Health Insurance:
– Blue Cross Blue Shield of Louisiana
– Coinsurance: 70% (In Network)/50% (Out of Network)
– Deductible: $500.00 (maximum of 3)
– Physician Co-pay: $20.00
– Pharmacy Co-pay: $7/$30/$70

Dental Insurance:
– Optional, paid by Employee
– Deductible: $50.00

Vision Insurance:
– Optional, paid by Employee
– Exams Copay: $10.00
– Materials Copay: $25.00
– Frames/Contact Lenses

Short Term Disability:
– Paid 100% by Black Hawk OFS
– Up to 60% of income for up to 3 months

Long Term Disability:
– Paid 100% by Black Hawk OFS
– Up to 60% of income until age 65

Life Insurance and AD&D
– Paid 100% by Black Hawk OFS
– $75,000 Life Insurance
– Up to $150,000 AD&D

401K
– Contribution up to federal allowed maximum
